Our office is open for outpatient appointments 8:00 AM – 4:00 PM Monday through Friday. We are closed for lunch from 12:00 until 1:00. Please schedule appointments and place non-emergency calls, including inquiries about medications, during regular business hours. Our staff is efficient, friendly, courteous and ready to help you with health insurance claims, appointments, and much more.

We make every effort to be on time and not make patients wait! We recognize that your time is as valuable as ours. It is essential that you arrive a few minutes early for your appointment, so that our staff can process necessary paperwork before the provider sees you.
Please be considerate of other patients: if you arrive late, it impairs our ability to see others on time. Occasionally, an emergency arises that does put us behind in the schedule. When this occurs, we make every effort to explain the situation to patients already here and to make their wait more comfortable.
We request 24 hours notice if you need to cancel or reschedule an appointment or a no-show fee may be applied. This insures that others needing attention may be seen promptly. Please bring your insurance card and co-pays with you to each appointment.
Often patients have accumulated many different prescriptions over time from multiple providers. Please bring to your appointment all medications that you currently have in your possession, so that we can determine (1) if each medication is still indicated, given your current medical condition; and (2) if there are potential adverse interactions between the medications that you have accumulated and may be taking.
Payment for office services is due when the service is rendered. We accept cash, money order, credit card (Visa, Master Card), or personal check.
Contact our office to verify that we participate with your insurance carrier. Insurers require that we collect co-payments. Full payment of applicable co-pays, deductibles and co-insurances is due at the time of the service. Our office will file insurance claims on your behalf. If you prefer to file your own claims, we will provide you with an itemized bill.
